Integrated Health Services ​

​An Integrated Health Home (IHH) is a team of professionals working together to provide whole person, patient-centered, coordinated care for adults with serious mental illness (SMI) and children with a serious emotional disturbance (SED). Currently serving Marshall, Hardin and Tama Counties.

Integrated Health Home (IHH)

An Integrated Health Home is a “home” for Care Coordinators, Medical Staff, Peer Supports and Family Supports, all working together to ensure individuals receiving Medicaid who have a serious mental illness (for adults) or serious emotional disturbance (for children) get the mental health/behavioral health care they deserve.  Once we take in a new client, we walk with them through the assessment, planning and funding process, and follow up regularly to ensure goals are met and services are delivered at the highest quality.
Community Support Services (CSS)
Community Support Services provides workers in the individual’s home and community who help break down barriers to mental health and healing.  From monitoring of mental health symptoms to crisis planning and support to building up natural support systems that help our clients get through their day, CSS is there to fill the gaps, and let the client focus on health.
